They are a discount plan whereby members in good standing are eligible to receive transparent discounts from the usual & customary fees that participating offices typically charge self-pay uninsured patients for treatment. Plan details and savings are exclusive to participating offices and may vary by location. Plan Forward, Inc. does not make payments directly to care providers for services rendered. Members are obligated to pay for all services they receive but will receive a discount on services rendered by participating care providers. The plan is not a qualified health plan under the Affordable Health Act. In Washington state, membership plans are to be referred to as discount plans.
